We, as a church, utilize this application and background-check process (for some teams) in order to create a safe, comfortable experience for every guest. Once we receive your application and background-check results (and you've been cleared to serve), you’ll hear from us about next steps to begin serving at Silverdale.

ADULTS: After completing this form, you will receive an email from our third-party, background-check partner, in order to complete a secure background check.

STUDENTS: After completing this form, the parent and recommendation contacts you provide will be contacted.
